A home visiting and care management service for families of high-risk infants
Taking a newborn home after days or weeks in the NICU can be an exciting time, but one also filled with anxiety.  Following Baby Back Home will make the transition easier for parents and their babies.
Home visitors provide teaching, support, and case management services for families of high-risk infants following their discharge from NICU.  The home visitors work closely with other service providers, such as home health or therapy services, to promote the very best start for the new baby and family.
Services provided are:  
  • Free of charge to the family
  • Continued as long as needed
  • Arranged to suit the family's schedule
  • Offered to the families taking their baby home from nurseries in the northeast, northwest, and southern regions of the state
  • Provided by home visitors who are registered nurses and professional care coordinators
  • Closely coordinated with the child's primary care provider and/or specialty clinics at ACH to assure continuity of care
Program offices are located in Morrilton, Pine Bluff, Lowell, Pocahontas, Little Rock, and Magnolia.
