What to Expect

What to Expect

  • Give your child all their medicine on the day of the appointment
  • Make sure your child eats breakfast
  • Bring any prior testing your child has had
  • If possible, do not bring any other children with you
  • Arrive 15 minutes early with your information packet completed (Do not mail or fax this packet ahead of time.)
  • Appointment times vary.  It is not unusual for appointment to last all day.  Be prepared to either bring a lunch or go out for lunch.
  • Be prepared to spend all day with your child.  We need your input too.  The person accompanying the child must be the legal guardian unless prior approval has been given in writing.
    Bring a snack for your child.
  • Make sure your child gets a good night’s sleep the night before their appointment.
  • If your child is ill (has fever, vomiting, or diarrhea) do not bring them for their appointment.  Please call and reschedule.

  • Completed Consent to Treat
  • Any Prior Testing
  • Snacks
